Am 16. Juli 1988 wird fast ganz Tokio in einer gewaltigen nuklearen Explosion vernichtet. Dies ist auch der Beginn des 3. Weltkrieges.

31 Jahre später, 2019, ist Tokio als Neo-Tokio wieder aufgebaut. In der düsteren Metropole wimmelt es von Gangs und Terroristen, die von der korrupten Regierung nur mühsam mit militärischer Gewalt unter Kontrolle gehalten werden können. Aufstände und brutale Bandenkriege prägen das Leben in den Straßen. Die Freunde Tetsuo und Kaneda sind zwei Jugendliche, die in Neo-Tokio leben. Schule und dergleichen interessieren sie nicht sonderlich, sie machen lieber mit ihrer Motorradgang nachts die Straßen unsicher und liefern sich auf ihren Bikes unerbittliche Kämpfe mit rivalisierenden Gangs. Eines Abends, als sie einen Kampf mit den »Clowns«, einer weiteren Gang austragen, trifft Tetsuo auf einen sonderbaren »Jungen«. Sein Körper sieht aus wie der eines Kindes, aber das Gesicht ist das eines alten Mannes. Kurze Zeit nach dem Zusammentreffen taucht das Militär auf, nimmt Tetsuo gefangen und führt später an ihm Experimente durch. Durch die Experimente hat Tetsuo starke psychische Kräfte erhalten, die er schon bald nicht mehr kontrollieren kann. Während Kaneda versucht Tetsuo zu befreien, verliert dieser immer mehr die Kontrolle über seine Kräfte.

Año 2019. Neo-Tokyo es una ciudad construida sobre las ruinas de la antigua capital japonesa destruida tras la Tercera Guerra Mundial. Japón es un país al borde del colapso que sufre continuas crisis políticas. En secreto, un equipo de científicos ha reanudado por orden del ejército un experimento para encontrar a individuos que puedan controlar el arma definitiva: una fuerza denominada "la energía absoluta". Pero los habitantes de Neo-Tokyo tienen otras cosas de las que preocuparse. Uno de ellos es Kaneda, un joven pandillero líder de una banda de motoristas. Durante una pelea, su mejor amigo, Tetsuo, sufre un extraño accidente y termina ingresado en unas instalaciones militares. Allí los científicos descubrirán que es el poseedor de la energía absoluta. Pero Tetsuo, que no se resigna a convertirse en un conejillo de indias, muy pronto se convertirá en la amenaza más grande que el mundo ha conocido.
